Introducción: En un servicio de anatomía patológica se analiza la carga laboral en tiempo médico en función de la complejidad de las muestras recibidas, y se valora su distribución entre los patólogos, presentado un nuevo algoritmo informático que favorece una distribución equitativa. Métodos: Siguiendo las directrices para la «Estimación de la carga de trabajo en citopatología e histopatología (tiempo médico) atendiendo al catálogo de muestras y procedimientos de la SEAP-IAP (2.ª edición)» se determinan las unidades de carga laboral (UCL) por patólogo y UCL global del servicio, la carga media laboral que soporta el servicio (factor MU), el tiempo de dedicación de cada patólogo a la actividad asistencial y el número de patólogos óptimo según la carga laboral del servicio. Resultados: Determinamos 12.197 UCL totales anuales para el patólogo jefe de servicio, así como 14.702 y 13.842 para los patólogos adjuntos, con una UCL global del servicio de 40.742. El factor MU calculado es 4,97. El jefe ha dedicado el 72,25% de su jornada a la asistencia y los adjuntos el 87,09 y 82,01%. El número de patólogos óptimo para el servicio es de 3,55. Conclusiones: Todos los resultados obtenidos demuestran la sobrecarga laboral médica, y la distribución de las UCL entre los patólogos no resulta equitativa. Se propone un algoritmo informático capaz de distribuir la carga laboral de manera equitativa, asociado al sistema de información del laboratorio, y que tenga en cuenta el tipo de muestra, su complejidad y la dedicación asistencial de cada patólogo.(AU)

We propose a computer algorithm capable of distributing the workload in an equitable manner. It would be associated with the laboratory information system and take into account the type of specimen, its complexity and the dedication of each pathologist to healthcare activity.(AU)

Antecedentes y objetivo: A nivel mundial la pandemia por el COVID-19 ha interrumpido el aprendizaje de más de mil millones de estudiantes, quienes reportan alteraciones en la salud mental, niveles altos de estrés académico y carga de trabajo. En la Licenciatura en Fisioterapia de la Universidad Nacional Autónoma de México, 331 estudiantes dejaron de asistir a clases presenciales indispensables para su formación académica y, tras un año de confinamiento, se desconoce el impacto psicológico y académico en esta población. El objetivo fue evaluar la salud mental, carga de trabajo y estrés académico de los estudiantes e identificar la asociación entre variables. Materiales y métodos: Se aplicó el Cuestionario de Salud Emocional por la Pandemia de COVID-19 - Fisioterapia, la escala de carga de trabajo UNIPSICO Battery y el Cuestionario de estrés estudiantil COVID-19. Se incluyeron los alumnos inscritos entre el primer y cuarto año de la Licenciatura en Fisioterapia de la Universidad Nacional Autónoma de México mayores de 18 años que contestaron la encuesta vía online y otorgaron su consentimiento informado. Resultados: El 62,7% de los estudiantes reportaron ansiedad, el 61,8% reportaron depresión, el 51,0% indicaron una percepción moderada del estrés académico y el 42,2% percibieron una carga de trabajo alta. La ansiedad y depresión tuvieron una asociación estadísticamente significativa con el estrés académico y carga de trabajo (p<0,05). Conclusiones: El estudio evidencia los efectos psicológicos y académicos derivados de la pandemia del COVID-19 en estudiantes de fisioterapia, que amerita medidas de intervención fisioterapéuticas y académicas que mitiguen los efectos del confinamiento.(AU)


Background and objective: Worldwide, the COVID-19 pandemic has disrupted the learning of over a billion students, who report mental health disorders, high levels of academic stress, and workload. In the bachelor's degree in physical therapy at the National Autonomous University of Mexico, 331 students stopped attending essential face-to-face classes for their academic training, and after a year of confinement, the psychological and academic impact on this population is unknown. The objective was to evaluate the mental health, workload, and academic stress of students in the bachelor's degree and identify the association between variables. Materials and methods: The Emotional Health Questionnaire for the COVID-19 pandemic – physical therapy was applied, including the UNIPSICO battery workload scale and the COVID-19 student stress questionnaire. Students enrolled between the first and fourth year of the Bachelor's degree in Physical Therapy at the National Autonomous University of Mexico over 18 years old who completed the survey online and provided informed consent were included. Results: 62.7% of students reported anxiety, 61.8% reported depression, 51.0% indicated a moderate perception of academic stress, and 42.2% perceived a high workload. Anxiety and depression had a statistically significant association with academic stress and workload (P<.05). Conclusions: The study demonstrates the psychological and academic effects of the COVID-19 pandemic on physical therapy students, which warrants physiotherapeutic and academic intervention measures to mitigate the effects of confinement.(AU)

Las fortalezas del carácter y la autorregulación de las emociones son recursos psicológicos relevantes para ayudar a los trabajadores a hacer frente a las demandas actuales en trabajo. La presente investigación tuvo como objetivo probar la asociación y el poder predictivo de las fortalezas del carácter con respecto a la autorregulación emocional. Un total de 203 participantes con edades entre 18 y 68 años respondieron el cuestionario sociodemográfico, la Escala de Fortaleza del Carácter - Breve (EFC-Breve) y la Escala de Autorregulación Emocional - Adulto (EARE-AD). Los resultados indicaron asociaciones de débiles a fuertes entre los factores CSS-Brief y ESRS-AD. La fortaleza intrapersonal y las fortalezas intelectuales e interpersonales predijeron las estrategias de autorregulación emocional susceptibles de ser adoptadas por los trabajadores. La evidencia recopilada sugirió diferencias significativas en ESRS-AD y CSS-Brief con respecto a las características sociodemográficas de los participantes. Estos resultados se han producido independientemente del nivel jerárquico y del departamento de trabajo de los trabajadores.(AU)


Character strengths and emotion self-regulation are relevant psychological resources to help workers cope with current demands in the workplace. The present investigation aimed to test the association with and the predictive power of the character strengths regarding emotion self-regulation. A total of 203 participants aged 18 to 68 years answered the so-ciodemographic questionnaire, Character Strength Scale –Brief (CSS-Brief), and the Emotion Self-Regulation Scale –Adult (ESRS-AD). The findings indicated weak to strong associations between the CSS-Brief and ESRS-AD factors. The intrapersonal strength and the intellectual and in-terpersonal strengths predicted the emotion self-regulation strategies likely to be adopted by the workers. The gathered evidence suggested significant differences in the ESRS-AD and CSS-Brief regarding the participants’ so-ciodemographic features. These results have occurred regardless of the workers’ hierarchical level and job department.(AU)

Background: Imagery rescripting (ImRs) has shown to be an effective treatment for posttraumatic stress disorders (PTSD) resulting from childhood-related trauma. The current theory is that the change of meaning of the trauma memory is central to the treatment. Several authors have suggested that the expression of needs, feelings and actions may act as potential healing factors, but little specific research aimed at (in)validating this hypothesis has been done so far.Objective: In this study we investigated to what extent the expression of inhibited action tendencies and the fulfilling of needs lead to the reduction of PTSD symptoms in clients with early childhood trauma.Method: Recordings of 249 therapy sessions of 24 ImRs treatments were rated with an observation instrument developed for this purpose, after which the scores were related to pre and posttreatment symptoms, assessed with the Impact of Events Scale-Revised (IES-R).Results: Scores on the IES-R decreased from pretreatment to posttreatment. The two subscales of the NATS (At-scale and N-scale)significantly predicted the posttreatment scores on the IES-R after controlling for the influence of pretreatment IES-R scores: the better the expression of inhibited action tendencies and the better the fulfilling of needs, the lower the symptoms after treatment.Conclusions: This pilot study on the underlying mechanisms of ImRs in PTSD treatment has shown that the expression of action tendencies and fulfilling basic needs during ImRs are associated with a decrease in PTSD symptoms after treatment, and that actions and basic needs cannot be viewed separately. Follow-up research could focus on which of the six domains of the Needs and Action tendencies Scale (NATS) has the greatest effect on the reduction of PTSD symptoms. With this information we can further improve the ImRs protocol.


The pilot study of working mechanisms of imaginary rescripting shows that the NATS is a reliable research tool for observing expressed action tendencies and fulfilled needs.The better the action tendencies are expressed during treatment and the better the needs are fulfilled, the lower posttreatment symptoms.It seems useful if practitioners are specifically trained during the ImRs training in performing actions that lead to the fulfilment of basic needs.

Objetivo: Caracterizar os aspectos trabalhistas das enfermeiras da Atenção Primária à Saúde nos municípios de tipologia urbana do estado da Bahia. Métodos: Estudo descritivo através de abordagem quantitativa a partir de informações obtidas com 32 enfermeiras, no período entre novembro de 2020 a maio de 2021, nos três maiores municípios do estado da Bahia. Resultados: Enfermeiras em sua maioria possuem vínculo trabalhista do tipo estatutário, com carga horária de 40 horas, atua há mais de um ano em unidades de saúde de modelo da Estratégia Saúde da Família, com diferenças salariais entre os municípios. Conclusão: Identificar aspectos trabalhistas das enfermeiras é de suma relevância, pois trazem implicações para melhoria das políticas de pessoal na Atenção Primária à Saúde. (AU)


Objective: To characterize the labor aspects of Primary Health Care nurses in urban municipalities of the state of Bahia. Methods: A descriptive study through a quantitative approach based on information obtained from 32 nurses, in the period between November 2020 and May 2021, in the 3 largest municipalities of the state of Bahia. Results: Most of the nurses have a statutory employment contract, work 40-hour shifts, and have been working for more than 1 year in health care units that follow the Family Health Strategy model, with wage differences among the municipalities. Conclusion: Identifying labor aspects of nurses is of utmost relevance, since they bring implications for improving personnel policies in Primary Health Care. (AU)


Objetivo: Caracterizar los aspectos laborales de las enfermeras de Atención Primaria de Salud en los municipios urbanos del estado de Bahia. Métodos: Estudio descriptivo a través de un enfoque cuantitativo basado en la información obtenida de 32 enfermeras, en el período comprendido entre noviembre de 2020 y mayo de 2021, en los tres mayores municipios del estado de Bahia. Resultados: La mayoría de las enfermeras tienen un contrato de trabajo legal, trabajan 40 horas y llevan más de 1 año en unidades sanitarias del modelo de la Estrategia de Salud Familiar, con diferencias salariales entre municipios. Conclusión: La identificación de los aspectos laborales de las enfermeras es de máxima relevancia, ya que aportan implicaciones para la mejora de las políticas de personal en Atención Primaria. Objetivo: descrever a experiência da reorganização do processo de trabalho de enfermeiros que atuam na Atenção Primária à Saúde no enfrentamento da covid-19, no município de João Pessoa - Paraíba. Métodos: Trata-se de um estudo descritivo com abordagem qualitativa, desenvolvido com um total de 15 enfermeiras atuantes no município de João Pessoa com idade entre 41 e 67 anos (média de 56 anos) e com tempo médio de 12 anos e 8 meses de atuação na Estratégia Saúde da Família. Resultados: foram evidenciados um campo amplo de alterações na dinâmica das Unidades Básicas de Saúde devido a pandemia, dentre elas, modificações no processo de trabalho da equipe, a estagnação das atividades coletivas, como também a implementação de ações destinadas para a covid-19. Conclusão: As experiências relatadas das participantes do estudo ressaltam o grande impacto que a pandemia trouxe para o cotidiano de uma equipe de Atenção Básica, sendo fundamental o fortalecimento da atenção primária à saúde, com recursos financeiros, incentivo e qualificação profissional, garantindo uma maior adequação da nova realidade vivida, a fim de garantir continuidade do cuidado. Objective: to describe the experience of the reorganization of the work process of nurses who work in Primary Health Care in the face of covid-19, in the city of João Pessoa - Paraíba. Objetivo: describir la experiencia de la reorganización del proceso de trabajo de los enfermeros que actúan en la Atención Primaria de Salud frente a la covid-19, en la ciudad de João Pessoa - Paraíba. Métodos: Se trata de un estudio descriptivo con enfoque cualitativo, desarrollado con un total de 15 enfermeros que actúan en la ciudad de João Pessoa con edades entre 41 y 67 años (promedio de 56 años) y con tiempo promedio de 12 años y 8 meses de vida. desempeño en la Estrategia de Salud de la Familia. Resultados: Se evidenció un amplio campo de cambios en la dinámica de las Unidades Básicas de Salud debido a la pandemia, entre ellos, cambios en el proceso de trabajo del equipo, el estancamiento de las actividades colectivas, así como la implementación de acciones dirigidas a la covid-19. Conclusión: Las experiencias relatadas por los participantes del estudio resaltan el gran impacto que la pandemia trajo para el cotidiano de un equipo de Atención Primaria, siendo imprescindible fortalecer la atención primaria de salud, con recursos económicos, incentivos y calificación profesional, asegurando una mayor adecuación. de la nueva realidad vivida, para garantizar la continuidad de la atención. Objetivo: analisar as práticas individuais e coletivas que estão associadas as dificuldades dos enfermeiros que atuam na Atenção Primária à Saúde da região Norte. Métodos: observacional de prevalência e analítico. Foram incluídos enfermeiros da atenção básica. A coleta ocorreu entre novembro de 2019 a agosto de 2021, nos sete estados do Norte, através de um formulário eletrônico, analisados pelos testes binomial, quiquadrado e G no programa Bioestat. Resultados: Entre os 626 enfermeiros do estudo, 15,7% (98/626) afirmaram ter dificuldade no exercício de suas práticas, quanto à autonomia das suas responsabilidades normativas legais. O Amapá teve menor proporção (8,3%; 9/108) de enfermeiros com dificuldade em relação a região. As práticas associadas a dificuldade foram: a não participação do enfermeiro no gerenciamento dos insumos (p=0,03), realizar consulta eventualmente (p=0,03) e nunca prescrever medicamentos (p=0,02); resolutividade insuficiente na consulta pré-natal (p=0,000), acompanhamento de crescimento e desenvolvimento infantil (p=0,001); planejamento familiar (p=0,000); hanseníase (p=0,005); tuberculose (p=0,031); hipertensão arterial (p<0,0001); diabetes (p<0,0001). Conclusão: As dificuldades quanto à autonomia das responsabilidades normativas legais estão associadas a práticas individuais e coletivas que são privativas do enfermeiro e estão regulamentadas nos programas de saúde pública. Objective: to analyze the individual and collective practices that are associated with the difficulties of nurses who work in Primary Health Care in the North region. Methods: observational of prevalence and analytical. Primary care nurses were included. The collection took place between November 2019 and August 2021, in the seven states of the North, through an electronic form, analyzed by the binomial, chi-square and G tests in the Bioestat program. Results: Among the 626 nurses in the study, 15.7% (98/626) stated that they had difficulty in exercising their practices, regarding the autonomy of their legal normative responsibilities. Amapá had a lower proportion (8.3%; 9/108) of nurses with difficulties in relation to the region. The practices associated with the difficulty were: the non-participation of nurses in the management of supplies (p=0.03), occasional consultations (p=0.03) and never prescribing medication (p=0.02); insufficient resolution in the prenatal consultation (p=0.000), monitoring of child growth and development (p=0.001); family planning (p=0.000); leprosy (p=0.005); tuberculosis (p=0.031); arterial hypertension (p<0.0001); diabetes (p<0.0001). Conclusion: Difficulties regarding the autonomy of legal normative responsibilities are associated with individual and collective practices that are exclusive to nurses and are regulated in public health programs. Introdução: As condições de trabalho de enfermeiros que atuam na Atenção Primária à Saúde se relacionam a questões de ordem interpessoal, política, organizacional e de infraestrutura. Objetivo: analisar a categoria "condições de trabalho" oriunda da pesquisa "Práticas de Enfermagem na Atenção Primária à Saúde: estudo nacional de métodos mistos", com foco nas capitais da região Sudeste do Brasil. Métodos: estudo qualitativo que analisou 108 entrevistas de enfermeiros residentes em Belo Horizonte, Rio de Janeiro, São Paulo e Vitória, realizadas entre novembro de 2020 e setembro de 2021. Os dados foram submetidos à análise temática, com apoio do software MAXQDA®. Resultados: os participantes eram majoritariamente do sexo feminino (88,0%), idade média de 42 anos, cor ou raça parda e tempo de atuação em atenção primária superior a 12 anos. Há uma diversidade temática quanto às condições de trabalho referidas por enfermeiros, destacando-se positivamente relacionamentos interpessoais. Infraestrutura das unidades de saúde e a fragilidade dos vínculos empregatícios foram referidos como dificultadores do trabalho na APS. Conclusão: Há uma diversidade de fatores que se relacionam com as condições de trabalho dos enfermeiros, sendo necessário intervenções sobre aquelas que trazem prejuízos, como o excesso de atribuições e infraestrutura das unidades de saúde. Background: The working conditions of nurses working in Primary Health Care are related to interpersonal, political, organizational and infrastructure issues. Objective: to analyze the category "working conditions" from the research "Nursing Practices in Primary Health Care: a national study of mixed methods", focusing on the capitals of the Southeast region of Brazil. Methods: qualitative study that analyzed 108 interviews with nurses residing in Belo Horizonte, Rio de Janeiro, São Paulo and Vitória, carried out between November 2020 and September 2021. Data were subjected to thematic analysis, with the support of the MAXQDA® software. Results: the participants were mostly female (88.0%), mean age of 42 years, brown color or race, and working in primary care for more than 12 years. There is a thematic diversity regarding the working conditions mentioned by nurses, with positive emphasis on interpersonal relationships. Infrastructure of the health units and the fragility of employment relationships were mentioned as obstacles to work in PHC. Conclusion: There are a variety of factors that are related to the working conditions of nurses, requiring interventions on those that bring harm, such as excess attributions and infrastructure of health units. Introducción: Las condiciones de trabajo de los enfermeros que actúan en la Atención Primaria de Salud están relacionadas con cuestiones interpersonales, políticas, organizativas y de infraestructura. Objetivo: analizar la categoría "condiciones de trabajo" de la investigación "Prácticas de Enfermería en la Atención Primaria de Salud: un estudio nacional de métodos mixtos", con foco en las capitales de la región Sudeste de Brasil. Métodos: estudio cualitativo que analizó 108 entrevistas con enfermeros residentes en Belo Horizonte, Rio de Janeiro, São Paulo y Vitória, realizadas entre noviembre de 2020 y septiembre de 2021. Los datos fueron sometidos a análisis temático, con el apoyo del software MAXQDA®. Resultados: los participantes eran mayoritariamente del sexo femenino (88,0%), edad media de 42 años, color moreno o raza, y actuaban en la atención primaria hace más de 12 años. Hay diversidad temática en cuanto a las condiciones de trabajo mencionadas por los enfermeros, con énfasis positivo en las relaciones interpersonales. La infraestructura de las unidades de salud y la fragilidad de las relaciones laborales fueron mencionadas como obstáculos para el trabajo en la APS. Conclusión: Hay una variedad de factores que se relacionan con las condiciones de trabajo de los enfermeros, requiriendo intervenciones sobre aquellos que traen perjuicio, como el exceso de atribuciones e infraestructura de las unidades de salud. Objetivo: Analisar o processo de trabalho das enfermeiras da Atenção Primária à Saúde frente à pandemia de COVID-19. Métodos: Trata-se de uma pesquisa qualitativa, descritiva e exploratória. A coleta de dados ocorreu de abril a maio de 2021, por meio de entrevista semiestruturada com 09 enfermeiras. Os dados foram analisados por meio de análise temática, e interpretados à luz da literatura pertinente. Resultados: Evidenciou-se o medo de faltar Equipamentos de Proteção Individual e recusa de atendimentos na falta destes. Utilizou-se a teleconsulta como meio de assistência de enfermagem, que mostrou limitações, como: invasão de privacidade das enfermeiras, extensão da jornada de trabalho e sobrecarga. Priorizou-se o atendimento à saúde da mulher e da criança. E ainda, notou-se o esforço no acolhimento aos casos suspeitos e agudos de COVID-19. Verificou-se a sobrecarga de trabalho e acúmulo de funções. Entretanto, lições foram adquiridas como a valorização de medidas de proteção, como a higiene das mãos. Conclusão: Infere-se a necessidade de criação e validação de novos protocolos que fortaleçam a assistência de enfermagem, bem como a importância de estimular as políticas de saúde para controle das Infecções Relacionadas à Assistência à Saúde. Objective: To analyze the work process of Primary Health Care nurses in the face of the COVID-19 pandemic. Methods: This is a qualitative, descriptive and exploratory research. Data were analyzed using thematic analysis and interpreted in light of the relevant literature. Results: The fear of lacking personal protective equipment was evidenced and the refusal of care in the absence of these. Teleconsultation was used as a means of nursing care, which showed limitations such as invasion of nurses' privacy, extension of the workday and overload. The health care of women and children was prioritized. Also, there was an effort to host suspected and acute cases of COVID-19. There was work overload and accumulation of functions. However, lessons were learned, such as valuing protective measures such as hand hygiene. Conclusion: The need to create and validate new protocols that strengthen nursing care is inferred, as well as the importance of stimulating health policies to control Health-Related Infections. Objetivo: Analizar el proceso de trabajo de las enfermeras de Atención Primaria de Salud frente a la pandemia de COVID-19. Métodos: Se trata de una investigación cualitativa, descriptiva y exploratoria. Los datos se analizaron mediante un análisis temático y se interpretaron a la luz de la bibliografía pertinente. Resultados: Se evidenció el miedo a la falta de equipo de protección personal y la negativa de atención en ausencia de estos. La teleconsulta fue utilizada como medio de atención de enfermería, que mostró limitaciones como la invasión de la privacidad de los enfermeros, la duración de la jornada laboral y la sobrecarga. Se priorizó la atención de la salud de las mujeres y los niños. Además, hubo un esfuerzo para albergar casos sospechosos y agudos de COVID-19. Había sobrecarga de trabajo y acumulación de funciones. Sin embargo, se aprendieron lecciones, como la valoración de medidas de protección como la higiene de manos. Conclusión: Se infiere la necesidad de crear y validar nuevos protocolos que fortalezcan los cuidados de enfermería, así como la importancia de estimular políticas de salud para el control de las Infecciones Relacionadas con la Salud. Objetivo: Analisar a percepção do enfermeiro acerca das condições de trabalho no contexto da Atenção Primária à Saúde (APS) durante a pandemia de SARS-CoV-2. Métodos: Trata-se de um estudo descritivo, transversal, de abordagem qualitativa, recorte de um estudo multicêntrico e de abrangência nacional. Os dados apresentados neste estudo correspondem aos obtidos no estado do Rio Grande do Norte, onde foram entrevistados 45 enfermeiros da APS no período de dezembro de 2020 a abril de 2021. Após as entrevistas, as narrativas gravadas foram transcritas e analisadas através da Análise de Conteúdo de Bardin. Resultados: A partir da fala dos enfermeiros, evidenciou-se que a utilização de Equipamentos de Proteção Individual, a organização e desenvolvimento de novos protocolos e fluxos assistenciais trouxeram sofrimento psíquico aos profissionais, tanto pelas condições de trabalho inadequadas, quanto pela proximidade com o sofrimento de pacientes e o medo de contágio. Conclusão: Mesmo com tantas dificuldades, os enfermeiros lotados na atenção primária à saúde tiveram que reconstruir suas práticas na perspectiva de garantir, dentro dos limites impostos pela situação adversa, a melhor assistência possível, mostrando a força e resiliência das equipes da APS nos mais diversos contextos. Objective: To analyze the perception of nurses about working conditions in the context of PHC during the COVID-19 pandemic. Methods: This is a descriptive, cross-sectional study with a qualitative approach, part of a multicenter study with a national scope. The data presented in this study correspond to those obtained in the state of Rio Grande do Norte, in which 45 nurses from primary health care were interviewed from December 2020 to April 2021. After the interviews, the recorded narratives were transcribed and analyzed through the Bardin's Content Analysis. Results: The collective discourse showed that the use of Personal Protective Equipment, the organization and development of new protocols and care flows brought psychic suffering to professionals, both due to inadequate working conditions and the proximity to the suffering of patients and the fear of Contagion. Conclusion: Even with so many difficulties, nurses working in primary health care had to rebuild their practices in order to guarantee, within the limits imposed by the adverse situation, the best possible assistance, showing the strength and resilience of PHC teams in the most diverse contexts. Objetivo: Analizar la percepción de los enfermeros sobre las condiciones de trabajo en el contexto de la APS durante la pandemia de COVID-19. Métodos: Se trata de un estudio descriptivo transversal con enfoque cualitativo, parte de un estudio multicéntrico de alcance nacional. Los datos presentados en este estudio corresponden a los obtenidos en el estado de Rio Grande do Norte, en el que fueron entrevistados 45 enfermeros de atención primaria de salud entre diciembre de 2020 y abril de 2021. Después de las entrevistas, las narraciones grabadas fueron transcritas y analizadas a través de la escala de Bardin. Análisis de contenido. Resultados: El discurso colectivo mostró que el uso de Equipos de Protección Individual, la organización y desarrollo de nuevos protocolos y flujos de atención trajeron sufrimiento psíquico a los profesionales, tanto por las inadecuadas condiciones de trabajo como por la proximidad al sufrimiento de los pacientes y el miedo al Contagio. Conclusión: Incluso con tantas dificultades, los enfermeros que actúan en la atención primaria de salud tuvieron que reconstruir sus prácticas para garantizar, dentro de los límites impuestos por la situación adversa, la mejor asistencia posible, mostrando la fortaleza y resiliencia de los equipos de APS en los más diversos contextos. OBJECTIVE: To analyze and compare the current Spanish and Chilean regulations regarding occupational risk prevention, regarding the existence of specific indications that protect the occupational health and safety of people over 55 years of age. METHOD: Qualitative study in which a documentary content analysis was carried out using ATLAS/ti. The sample was 88 regulatory documents on occupational risk prevention for Spain and Chile. The guidance of the European Agency for Safety at Work (EU-OSHA) regarding age-critical risks was followed. RESULTS: In global terms, 21.9% of the total Spanish documents analyzed show the explicit presence of considerations on aging, while for Chile this occurs in 9%. Both countries mention indications regarding ergonomic risks and noise. Shift work and psychosocial risks are considered only in Spain, while extreme temperatures, vibrations and geographical altitude appear exclusively in Chilean regulations. Neither country refers to issues inherent to working women (care responsibilities, menopause). CONCLUSIONS: The Spanish regulations present a greater presence of indications on aging compared to the Chilean one. However, development in this area is insufficient in both countries. Chile and Spain have guides of recommendations, which are not mandatory, and their application is voluntary. This suggests that the prevention of occupational risks has a great pending challenge with people over 55 years of age.

Introducción: El objetivo es lograr una plantilla de mapeo de activación cerebral promedio en niños sanos usando la resonancia magnética funcional (RMf), con paradigmas específicos para activar funciones de atención inhibitoria y de memoria de trabajo. Sujetos y métodos: Se realizó una evaluación nutricional y neuropsicológica a 87 niños diestros. Cumplieron los criterios de inclusión 30 (15 niños y 15 niñas) entre 9 y 11 años, a quienes se estudió con RMf con realización de dos pruebas de atención inhibitoria (Go/No Go), con letras e imágenes de alimentos, un test de memoria de trabajo (Test de Atención Continua-Pares Idénticos) y obtención de volúmenes anatómicos. Los datos posteriormente se procesaron con el programa FSL-v5 con un umbral de p < 0,05 (cluster-wise). Las áreas cerebrales activadas se localizaron utilizando una plantilla cerebral estándar del Montreal Neurological Institute y el atlas cortical estructural de Harvard-Oxford. Resultados: En las pruebas de atención inhibitoria hay activación en áreas frontales de predominio derecho, cíngulo, parietales y occipitales, con preponderancia en áreas occipitales en la prueba con alimentos. En la prueba Test de Atención Continua-Pares Idénticos se obtuvo activación de predominio en áreas occipitales, frontales y parietales. Conclusiones: Se obtienen plantillas de mapeo de actividad cerebral en niños sanos con test de atención inhibitoria, de alimentos y de memoria de trabajo. Las áreas de activación corresponden mayoritariamente a las descritas en la bibliografía. Esto nos permite tener patrones basales de activación cerebral para estudiar patologías relacionadas con la atención inhibitoria, la impulsividad y la memoria de trabajo.(AU)


Introduction: The objective is to produce an average brain activation mapping template in healthy children using functional magnetic resonance imaging (fMRI), with specific paradigms for activating inhibitory attention and working memory functions. Subjects and methods: A nutritional and neuropsychological evaluation was performed on 87 right-handed children. The inclusion criteria were met by 30 children (15 boys and 15 girls) between 9 and 11 years old, who were studied with fMRI in two inhibitory attention tests (Go/No Go), with food cues, a working memory test (Continuous Performance Test Identical Pairs) and measurement of anatomical volumes. These data were subsequently processed with the FSL-v5 program, with a threshold of p < 0.05 (cluster-wise). The brain areas activated were located using a standard Montreal Neurological Institute brain template and the Harvard-Oxford structural cortical atlas. Results: The inhibitory attention tests showed activation frontal areas predominantly on the right, and the cingulate, parietal and occipital areas, with preponderance in occipital areas in the food cues test. In the Continuous Performance Test-Identical Pairs test, activation was obtained predominantly in the occipital, frontal and parietal areas. Conclusions: Brain activity mapping templates are obtained in healthy children with tests for inhibitory attention, food cues and working memory. The activation areas are mostly those reported in the literature. This provides baseline brain activation patterns for studying pathologies related to inhibitory attention, impulsivity and working memory.(AU)

Fundamentos: las infecciones de transmisión sexual (its) son infecciones causadas por más de treinta bacterias, virus y pa-rásitos diferentes, que se transmiten por contacto sexual, incluido el coito vaginal, anal o bucal, aunque algunas infecciones también pueden transmitirse de la madre al hijo durante el embarazo, el parto y la lactancia. Según la organización mundial de la salud, una de las principales poblaciones de riesgo para la adquisición de estas infecciones son los/las trabajadores/as del sexo y sus clientes. El objetivo de este trabajo fue analizar los factores de riesgo en la adquisición de infecciones de transmisión sexual en la población de trabajadores/as del sexo.métodos: se realizó una revisión sistemática mediante búsqueda en scopus, pubmed, cinahl, medline, lilacs e ibecs, de estudios observacionales, longitudinales y mixtos realizados en trabajadores/as del sexo que fueron publicados entre enero de 2011 y marzo de 2021, obteniendo una muestra final de cuarenta y tres artículos. La herramienta de evaluación de métodos mixtos (mixed methods assessment tool) fue utilizada para evaluar la calidad metodológica de los trabajos. Resultados: después de analizar la bibliografía se obtuvieron once grupos de factores de riesgo relacionados con la adquisición de its, siendo estos factores económicos, relacionados con las prácticas sexuales, consumo de drogas, trabajo sexual, educación, problemas de salud, país de origen y movilidad, pareja, edad, violencia y otros factores no pertenecientes a las anteriores categorías.conclusiones: las/os profesionales del sexo tienen numerosas características y comportamientos que les convierten en una población vulnerable a las its. Destacan los años ejerciendo la prostitución, el consumo de drogas, el número de parejas y el uso del preservativo, siendo estos factores clave para futuras estrategias de prevención e intervención, así como de investigación.(AU)


Background: sexually transmitted infections (sti), as their name suggests, are infections caused by more than thirty different bacteria, viruses, and parasites and are transmitted through sexual contact, including vaginal, anal, or oral intercourse, although some sexually transmitted infections can also transmitted from mother to child during pregnancy, childbirth and lactation. According to world health organization, one of the main populations at risk for acquiring these diseases are sex workers and their clients. Due to the high prevalence, the characteristics and behaviors that favor the development of these diseases are analyzed. The main objective of this paper was to analyze the risk factors in the acquisition of sexually transmitted diseases in the population of sex workers. Methods: a literature search was conducted in scopus, pubmed, cinahl, medline, lilacs and ibecs, of observational, longitudinal and mixed methods studies conducted in sex workers and published between january 2011 and march 2021, with a final sample of four-ty-three articles. The mixed methods assessment tool (mmat) was used to assess the methodological quality of the papers.results: after analyzing the literature, eleven groups of risk factors related to the acquisition of sexually transmitted diseases were obtained, these being economic factors, factors related to sexual practices, drug use, sex work, education, health problems, country of origin and mobility, partner, age, violence and other factors not belonging to the previous categories.conclusions: sex workers have numerous characteristics and behaviors that make them vulnerable to sexually transmitted in-fections. Of note are years of prostitution, drug use, number of partners and condom use, which are key factors for future prevention and intervention strategies, as well as research.(AU)

Introducción: El embarazo representa un estado procoagulante que aumenta el riesgo de fenómenos trombóticos en la mujer. Al igual que en la población no gestante, deberemos diagnosticar y tratar lo más precozmente posible dichos eventos para evitar la migración del trombo a otras localizaciones. La anticoagulación constituye la medida terapéutica de primera línea. Sin embargo, ciertas situaciones de aumento de sangrado como el trabajo de parto pueden suponer una contraindicación para iniciar dicha anticoagulación. En el siguiente artículo se desarrolla un caso clínico en el que el diagnóstico de trombosis venosa profunda tuvo lugar al inicio del trabajo de parto. Principales síntomas y/o hallazgos clínicos: Aumento de diámetro de miembro inferior izquierdo, con enrojecimiento del mismo y aumento de temperatura local. La paciente describía dolor a nivel de dicha extremidad desde hacía 15 días. No presentaba disnea y la saturación de oxígeno era del 100%. Diagnósticos principales: Trombosis venosa profunda, trabajo de parto. Intervenciones terapéuticas: La contraindicación de anticoagulación y la necesidad de un tratamiento inmediato plantearon el filtro de vena cava inferior como la opción más razonable. Resultados: Tras la colocación del filtro, el parto cursó sin incidencias; no migró el émbolo y el filtro se retiró a los 29 días del parto sin complicaciones. Conclusión: Pese a nuestra escasa experiencia clínica en el manejo de estos casos, podemos afirmar que el filtro de vena cava inferior representa una opción razonable y segura en los casos de trombosis de miembros inferiores en mujeres en trabajo de parto.(AU)


Introduction: Pregnancy represents a procoagulant state that increases women's risk of thrombotic phenomena. As in the non-pregnant population, we must diagnose and treat these events as early as possible to avoid the migration of the thrombus to other locations. Anticoagulation is the first-line therapeutic measure. However, certain situations of increased bleeding such as labor may be a contraindication to initiate such anticoagulation. The following article develops a clinical case in which the deep vein thrombosis diagnosis occurred at the onset of labor. Main symptoms and/or clinical findings: Increase in diameter of the left lower limb, with redness of the same and increase in local temperature. The patient described pain at the level of this limb for 15 days. He had no dyspnea and oxygen saturation was 100%. Main diagnoses: Deep vein thrombosis, labor. Therapeutic interventions: The contraindication of anticoagulation and the need for immediate treatment raised the inferior vena cava filter as the most reasonable option. Results: After the placement of the filter, the delivery proceeded without incident. The plunger did not migrate and the filter was removed 29 days after delivery without complications. Conclusion: Despite our limited clinical experience in the management of these cases, we can affirm that the inferior vena cava filter represents a reasonable and safe option in cases of thrombosis of the lower limbs in women in labor.(AU)

Introduction: Ground transportation drivers face substantial risks to cardiovascular health, mainly hypercholesterolemia, possibly related to the hours of the workday.Objective. Determine the relationship between working hours and cholesterol level in land transport drivers in a company in Lima between the period from January 2022 to August 2023.Materials and methods. Correlational, cross-sectional research, which included 107 transport drivers. The main variables were: Hours of daily and weekly workday, total cholesterol and high-density lipoproteins (HDL), the instrument used was a data collection form. Spearman's Rho correlation test was used in the statistical analysis. The study was approved by the Ethics Committee of the Universidad Nacional Mayor de San Marcos.Results. It was observed that 47.02% of the workers worked between 9 and 10 hours a day, while 57.9% worked 61 to 72 hours a day. Furthermore, the majority of drivers had total cholesterol levels between 160 and 199 mg/dl (38.3%), and HDL cholesterol between 35 and 59 mg/dl (93.5%). A significant and direct correlation was determined between daily (p=0.026, Rho=0.216) and weekly (p=0.038, Rho=0.201) working hours with total cholesterol. No significant relationship was found with HDL cholesterol.Conclusions. It was identified that the hours of daily and weekly work hours were significantly related to the level of total cholesterol.


Introducción: Los conductores de transporte terrestre enfrentan riesgos sustanciales para la salud cardiovascular, principalmente hipercolesterolemia, posiblemente relacionadas a las largas horas de jornada laboral.Objetivo. Determinar la relación entre las horas de jornada laboral y nivel de colesterol en conductores de transporte terrestre en una empresa de Lima entre el periodo de enero 2022 hasta agosto de 2023.Materiales y métodos. Investigación correlacional, de corte transversal, que incluyó a 107 conductores de transporte. Las variables principales fueron: Horas de jornada laboral diaria y semanal, colesterol total y lipoproteínas de alta densidad (HDL), el instrumento empleado fue una ficha de recolección de datos. En el análisis estadístico se empleó la prueba de correlación Rho de Spearman. El estudio fue aprobado por el Comité de Ética de la Universidad Nacional Mayor de San Marcos.Resultados. Se observó que el 47,02% de los trabajadores laboraba entre 9 y 10 horas diarias, mientras que el 57,9% lo hacía de 61 a 72 horas. Además, la mayoría de los conductores presentaba niveles de colesterol total entre 160 y 199 mg/dl (38,3%), y colesterol HDL entre 35 a 59 mg/dl (93,5%). Se determinó correlación significativa y directa entre las horas de jornada laboral diaria (p=0,026, Rho=0,216) y semanal (p=0,038, Rho=0,201) con el colesterol total. No se encontró relación significativa con el colesterol HDL.Conclusiones. Se identificó que las horas de jornada laboral diaria y semanal se relacionaron significativamente con el nivel de colesterol total.

Introducción: Los trabajadores de salud, especialmente enfermeras y enfermeros, laboratoristas y paramédicos tienen mayor riesgo de exposición a lesiones por pinchazo de aguja, en ocasiones, por un inadecuado manejo y almacenamiento de desechos hospitalarios. Objetivo: Describir las causas de lesión por pinchazo de aguja y las medidas de prevención tomadas por los trabajadores de salud. Métodos: Se realizó un estudio de corte transversal utilizando una encuesta en línea; participaron 74 trabajadores de salud del Hospital Nacional Almanzor Aguinaga Asenjo y el Hospital Regional Lambayeque; los datos están presentados como estadística descriptiva; para el análisis estadístico se utilizó InfoStat 2019. Resultados: La edad media de los encuestados fue de 28,9 años (rango de 17-48); la mayoría fueron mujeres (56,8 %), y el 43,2 %, varones. Del total de encuestados (42, 56,8 %), alguna vez tuvo una lesión por pinchazo de aguja, mientras, el 43,2 % no ha recibido lesiones. La severidad del pinchazo de aguja fue leve en 51,4 % de los casos, 6,8 % fue moderada y 41,9 % no tuvo lesión. Conclusiones: Las principales modalidades causantes de pinchazo de aguja encontradas en este estudio fueron: reencapuchar la aguja, extracción de sangre venosa, el movimiento repentino del paciente durante una inyección o una sutura. Una capacitación continua, el uso de equipos de protección personal y seguir las pautas de eliminación adecuada de los objetos cortopunzantes, ayudarían a prevenir este tipo de lesiones en trabajadores de salud y por ende, evitar la transmisión de infecciones a través de sangre percutánea.


Introduction: healthcare workers, especially nurses, laboratory workers and paramedics have a higher risk of exposure to needle-stick injuries, which is sometimes due to inadequate handling and hospital waste storage. Objective: to describe causes of needle-stick injury and preventive measures taken by healthcare workers. Methods: a cross-sectional study was conducted using an online survey; 74 healthcare workers from the Almanzor Aguinaga Asenjo National Hospital and the Lambayeque Regional Hospital participated; data are presented as descriptive statistics; InfoStat 2019 was used for the statistical analysis. Results: the mean age of the respondents was 28.9 years (range 17-48); the majority were female (56.8%), and 43.2%, male. The 56.8% from the total number of respondents (42) had ever had a needle-stick injury, while 43.2% had not received injuries. The severity of the needle stick was mild in 51.4% of the cases, 6.8% was moderate and 41.9% had no injury. Conclusions: needle recapping, venous blood withdrawal and patients' sudden movement during an injection or a suture were the main causes of needle stick found in this study. A continuous training, the use of personal protective equipment, and following the proper disposal guidelines for sharp objects would help health workers to prevent this type of injury, and thus to avoid the transmission of infections through percutaneous blood.

RESUMO

This article characterizes the main features of the supply, demand, and labor markets for physicians in Spain, with an international and territorial perspective. It also presents some of the results of the simulation model for specialist physicians with a 2035 horizon and proposes strategic and short-term lines of action in the planning, regulation, and management of health professionals in Spain, with a focus on specialist physicians. In Spain there are high rates of physicians and medical graduates, but low rates of nurses, compared to other developed countries. Approximately 30% of practicing physicians (not considering residents) practice only in the private network. In the last two decades, competition from the private sector for health professionals has intensified, competing with the public network, which is subject to an excessively rigid regulatory framework. There is currently a shortage of physicians in some specialties, particularly in family medicine, which urgently needs specific incentives to stimulate vocations. Numbers consider only part of the story. The imbalances in the educational and labor markets are not resolved by creating vacancies, but by reforming the regulatory framework, the incentive systems, and the slack in public management to compete with the private sector in attracting and retaining talent.

Fundamento. El objetivo de este estudio es analizar la asociación entre la patologia funcional tiroidea y la exposición a turnicidad/nocturnidad laboral, y describir los trastornos tiroideos más prevalentes por turno de trabajo. Metodología. Estudio transversal realizado en un servicio de urgencias hospitalario de Almería (España). Se relacionaron los niveles de tiroxina y tirotropina (TSH) con el turno, la categoría profesional y los antecedentes de patologia tiroidea. Resultados. Se incluyeron 133 trabajadores, 80,5% mujeres, edad media 46,11 años (38 a 65) y 52% personal enfermero; las mujeres mostraron más frecuentemente antecedentes tiroideos. El 81,2% trabajaba en turno rotatorio y el 11,3% en turno nocturno (12,1% de mujeres y 7,7% de hombres). El 27% mostró alteraciones tiroideas, más frecuentemente niveles elevados de TSH con niveles normales de tiroxina, especialmente en turno nocturno (61,1%). Las alteraciones de TSH fueron más frecuentes en turno nocturno que en rotatorios (53,3 vs 13,0%; p<0,001). El turno nocturno presentó valores medios de TSH en rango normal pero significativamente superiores al resto de turnos, mientras los niveles de tiroxina fueron similares. El turno diurno no presentó alteraciones. El turno nocturno y la presencia de antecedentes fueron predictores independientes de presentar alteraciones tiroideas. Conclusiones. El turno nocturno y los antecedentes de patología tiroidea fueron más frecuentes en mujeres, y ambos se asociaron con la presencia de alteraciones tiroideas, indicando la necesidad de incluir la evaluación de dichas alteraciones en los programas de vigilancia de salud y de analizar las diferencias por sexo. Background. The study aim was to analyze the relationship between functional thyroid pathology and the exposure to work shifts / night shifts, and describe the most prevalent thyroid disorders based on the type of shift. Methodology. Cross-sectional study performed in the emergency department of a hospital in Almeria (Spain). Relationships between thyroxine and thyrotropin levels (TSH) and work shifts, professional category and history of thyroid pathology were analyzed. Results. The study included 133 workers; 80.5% female, average age was 46.11 years (38 – 65), and 52% were part of the nursing staff; thyroid disorders were more frequent in female participants. Most participants (81.2%) had rotating shifts schedules and 11.3% night shifts (12.1% female and 7.7% male). Thyroid alterations were found in 27% of the participants (usually elevated TSH levels and normal thyroxine levels), particularly in those doing night shifts (61.1%). TSH alterations were more frequent in individuals doing night shifts than in rotating shifts (53.3 vs 13.0%; p<0.001). Individuals working night shifts had mean TSH values in the normal range, although significantly higher than the individuals in the rest of the shifts; thyroxine levels were found to be similar. No thyroid disorders were found in day shift participants. Night shift and a history of thyroid pathology were independent predictors of thyroid disorders. Conclusions. Night shift schedules and history of thyroid disorders are more frequent in female, both related to the presence of thyroid disorders, indicating the need to include the evaluation of these disorders in health surveillance programs and analyze gender differences. Fundamento. La vida laboral del policía es estresante dada la exposición al riesgo y a la violencia en el trabajo. Según el Modelo de Demandas y Recursos laborales (DRL), las condiciones de trabajo de los policías pueden influir en la salud y la satisfacción laboral. Los objetivos de esta investigación fueron evaluar los factores psicosociales en el trabajo en una muestra de policías municipales de la Comunidad Valenciana (España), identificar la prevalencia de riesgos psicosociales y sus consecuencias, y analizar la influencia de los factores psicosociales (recursos y demandas) en el trabajo sobre la satisfacción laboral y la salud. Material y método. Se utilizó la Batería UNIPSICO para evaluar la prevalencia y la distribución de policías (n=103) en los diferentes niveles de riesgo y consecuencias. La influencia de los factores psicosociales (recursos y demandas) en el trabajo sobre los trastornos psicosomáticos y la satisfacción laboral se examinó mediante un análisis de regresión por pasos. Resultados. Se obtuvieron valores moderados de riesgo psicosocial en todas las variables para toda la muestra. Para las consecuencias de los riesgos psicosociales también se obtuvieron niveles moderados de trastornos psicosomáticos y satisfacción laboral. Los niveles de estrés percibidos y los estresores organizacionales tienen un efecto negativo sobre la salud y la satisfacción laboral. Conclusión. Para facilitar condiciones positivas para la salud y el bienestar de los policías, se requiere un compromiso real para la evaluación de riesgos psicosociales en las organizaciones policiales locales y para la adopción de medidas de prevención e intervención, donde se detecten factores de riesgo psicosocial. Background. Police officers are exposed to risk and violence, which makes their work highly stressful. According to the Job Demand-Resource model, the working conditions of police officers may affect their health and job satisfaction. The aims of this research were to evaluate the psychosocial factors at work in a sample of municipal police officers from the Valencian Community (Spain) to identify the prevalence of psychosocial risks and their consequences and analyse the influence of psychosocial factors (resource and demands) at work on job satisfaction and health. Methods. The Unidad de Investigación Psicosocial de la Conducta Organizacional (UNIPSICO) battery was used to evaluate the prevalence and distribution of different levels of risk and their consequences in police officers (n=103). The influence of psychosocial factors (resource and demands) at work on psychosomatic disorders and job satisfaction was examined using stepwise regression analysis. Results. All considered variables showed moderate levels of psychosocial risk for the whole sample. As for the consequences of psychosocial risks, moderate levels of psychosomatic disorders and job satisfaction were also observed. Perceived stress levels and organizational stressors have a negative effect on health and job satisfaction. Conclusions. To create positive conditions for the health and well-being of police officers, real commitment is required when evaluating psychosocial risks in local police organizations and the establishment of prevention and intervention measures where psychosocial risk factors are detected.
